This learning activity explores the principles and practice of building effective collaborative health teams in school settings, equipping school nurses with the frameworks, tools, and strategies needed to coordinate care across professional boundaries. Drawing on the NASN Framework, the WSCC model, and MTSS, it examines the school nurse's role as care coordinator, advocate, and population health resource while addressing mental health integration, health equity, community partnerships, and infection prevention. Practical tools for immediate implementation are included throughout.
